Delayed breast reconstruction after mastectomy for breast cancer. Breast was rebuilt with tissue from abdomen and DIEP flap. The vessels nourishing the flap were dissected through the muscle which allowed abdominal strength to be preserved. The abdominal scar looks as after cosmetic tummy tuck. Six months down the line the patient had the second stage of her reconstruction. The nipple was rebuilt from the skin of the flap and small volume fat grafting was performed to the upper pole to improve the symmetry. The patient also had the areola tattooed.
